Medical records from closed clinic now available, here is how to get them

Here is the summary:

1) Open weekends only. See below for hours.
2) Fill out the release form. Available in-person, online, or via e-mail.
3) Can’t make it on weekends? Designate a spouse, partner or parent to pick up the records.
4) Transferred to another doctor? For certain doctors, have your physician request to directly transfer the records.
5) Moved? You can request mail delivery.
6) Please donate to help to defray costs.



If your records are among those currently in storage, your medical records will be
available for pickup
at the office of Purisima Family Medicine, 575 Kelly Ave, HMB,
on weekends only – please do not disturb the office staff during the week. For February,
we will be open Saturdays & Sundays, 10am-4pm, closed for lunch from 12:45-
1:15pm. From March until mid-August, we will be open Sundays only, same times.
Please follow the procedure below to obtain your records.

Either simply email names & birthdates to and fill out the
release form when picking up, or, to save time complete a release form first, which will
be available at the office during the above times, or online at, for yourself and your family, and bring with you. You may also return the form to the office during weekend hours, fax it to 650-318-3634,
or mail it to P.O. Box 370051, Montara, CA 94037. Please include an email address or
phone number on your form

Spouses/partners & adult children who are unable to visit the office during pickup hours
may designate their spouse/partner or parent to pick up their records.
Patients who have transferred their care to a local Coastside physician (Drs. McMillan,
Pavliscak, Oppenlander, Quest, Enriquez, Penrose, Carrington, Lessani or Lu only) may
have their new physician’s office request their records from us directly, and these will be
delivered during the week to their offices – please check with your doctor’s office, some
may wish you to pick them up and bring with you on your next visit. Unfortunately we
are unable to hand-deliver records to out of area physicians.

Patients who have moved out of the area may request mail delivery – we will need the
release form and a copy of photo ID – we will pack your records and have them weighed
for delivery via USPS media mail with signature confirmation, and then request payment
of the cost of shipping – once received they will be mailed to you.

Once your information has been received your records will be retrieved from storage and
available for pickup on the following schedule – requests received before Wednesdays
at 9am will be available that weekend; requests received after Wednesdays at 9am
will be available the following weekend. Please bring a photo ID

This will be a free service, however in order to cover our expenses we would appreciate a
small voluntary tax-deductible contribution of $5 per record if you are able.

Please help spread the word to any former patients you know, especially those who may
have moved out of the area. We only have until mid-August, after which remaining
